RADIAS Health has a full-time Program Director role on our Housing Navigation team. The Housing Navigation Team provides outreach, housing transitional services, and tenancy sustaining services to qualifying individuals in Ramsey and Anoka Counties. Work involves the identification, development, implementation and evaluation of program policies and procedures as directed by state, federal and administrative codes, as well as professional practice standards. Work also involves direct client contact and clinical supervision of Housing Navigation Team members. Duties include, being responsible for all clinical, organizational, and financial aspects of the program, providing consultation and training to all program staff regarding implementation of structure and systems pertaining to the program, approving and evaluate all individualized Housing Support Plans and Housing Support Crisis Plans, conducting diagnostic assessments, coordinating of staff coverage, Consultation and development of interventions, training and supervising staff, including performance evaluations, growth and development plans, and discipline.
Duties & Responsibilities
Responsible for all clinical, organizational, and financial aspects of the program.
Develop and maintain program policies and procedures and revise as necessary.
Provide consultation and training to all program staff regarding implementation of structure and systems pertaining to the program.
Ensure standards are met according to State and County guidelines and ensure appropriate reporting is submitted to Vulnerable Adult, Child Protection, the Ombudsman’s Office, and other agencies/entities are made when necessary.
Audit of program (quality assurance, chart audits, attending staff meetings, etc.)
Approve and evaluate all individualized Housing Support Plans and Housing Support Crisis Plans at least quarterly.
Continually monitors client progress and with the clinical team and modifies the Housing Support Plan and Housing Support Crisis Plan as appropriate. Director communicates changes in service needs and recommendations to the treatment team on an ongoing basis to coordinate client care.
Conduct Diagnostic Assessments and other relevant assessments as necessary
Consultation and development of interventions.
Coordination of staff coverage
Assist Director and Associate Director of Clinical Services in the development and update of contract with Department of Human Services
Accountable for DHS, MA and/or County inspections of program.
Act as designated Responsible Authority for data privacy purposes.
Investigate and report suspected incidents of abuse and neglect.
Public Relations and Marketing duties to referral sources as necessary.
Serve on work related community service committees, advisory boards, etc.
Submit management/financial reports to Director.
Statistics and Data Collection and submit demographic data pertaining to target population as assigned.
Hiring of all positions in concert with Human Resources Director and Recruiter.
Supervision of all staff, including performance evaluations, growth and development plans, discipline and recommendations for salary adjustments.
Attend relevant meetings, conferences, and training sessions outside of facility.
Responsible for review and resolution of resident and employee grievances.
Ability to interpret statistical and evaluation reports.
Ensures that required program evaluations are completed.
Develop and ensure completion and compliance of all mandated training. Identify and/or conduct any necessary individual training for direct reports.
Ensure compliance with any training mandates.
Schedule and document all training sessions and hours appropriately.
Responsible for keeping on-going performance documentation for all direct reports.
Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
Maintain a pattern of regular work hours.
Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
Other duties as assigned.
A Master’s degree in the behavioral sciences or related field.
Minimum of 4,000 hours of work experience providing services to mentally ill persons.
2,000 hours of experience or training in administration or supervision.
Must be a qualified Mental Health Professional, LICSW, LMFT or LP.
